Do you have a chronic medical condition requiring you to see a health care professional more than once per month? Do you need help managing your high blood pressure or diabetes? This monthly plan is designed to help you manage your health without additional strain on your finances. The monthly service also covers basic labs. Contact us today for further questions to see if this plan is a great supplement to your current insurance. 

 

*This does not replace health insurance. It is a supplement to your current health plan enabling your basic medical care to be taken care of without additional cost. Complex labs, referral to specialist, etc may incur additional costs.*

Are telehealth visits available?
Yes. We understand life can get busy, and we understand sometimes it can be physically hard to come to a doctor’s office. For your convenience we also offer virtual visits. If you choose the Direct Primary Care Membership, this includes unlimited monthly in office or telehealth visits. Telehealth may not be suitable for every patient though. We will discuss at your initial appointment to see if telehealth appointments could work for you. 
Do you see pediatric patients?

Direct Primary Care starts at age 16. This is due to the vaccinations children require up through this age are best regulated for timing through their pediatrician.

Is there a contract for Direct Primary Care?

Yes, there is a 3 month minimum for the Direct Primary Care Membership. 

What types of things does the Direct Primary Care Membership cover?

Chronic illnesses that need regular check ins and follow ups. If you are a diabetic or a patient with high blood pressure for example, those conditions must be monitored regularly to ensure your levels are stable and to address any medication needs or changes. High cholesterol patients are another good fit for routine primary care.
